Continuation of Services Examples
Samples of possible supporting documentation for each response are provided below. These are examples and not inclusive of all possible responses.
Example 1a: Letter offering Continuation of Services that shows Parent declined services.

As you are aware, a current IEP exists for your son. According to Georgia law, after a child with a disability has been removed from his current placement for more than 10 school days in the same year because of disciplinary removals, the school district must provide services to enable the student to continue to make progress towards IEP goals.
The most recent disciplinary incident provided a need for additional out of school suspension days resulting in greater than ten school days for the school year. We need to arrange a time for a certified special education teacher to work with your child off campus.
Please indicate your choice by selecting one of the boxes below identifying if you would like your son to receive services while he is suspended. Please contact the Exceptional Student Director at ABC County Board of Education at (555) 555-5555 to arrange services or if you have any additional questions or concerns.

We still have availability to work with her this summer if you reach out to me.

Additional documentation provided included:
- Telephone log showing phone call with parent on 7/30/XX. Parent informed the LEA that she was withdrawing to attend Job Corps.

Example 2. Continuation of Services was provided but not reported. * (Services were provided during the period of exclusion but not documented with a “Y”- in the SIS. Documentation would be similar to the examples in 1a-c.)

Example 3: Continuation of Services was not offered; the student was not suspended for 11 days. *
Documentation should include, but not limited to:
- IEPs with meeting notes from this school year
- Power School/Infinite Campus or SIS information system showing student’s attendance and discipline record.
- The SIS Discipline and attendance record for the student should match with IEP documentation.







Example 4: Continuation of Services was not offered, the student was not suspended for 11 days, but the student was not a SWD or suspected SWD for 11 or more suspension days. **
Documentation may include, but not limited to:
- IEP meeting (s) minutes
- Eligibility or re-eligibility meetings with notes
- Discipline record that showed the student did not go over 10 days after initial provision of special education services or over 10 days before the revocation of services and completed PWN.
- Include either consent for services or revocation in the supporting documentation.
- Attendance records from the SIS.
Example 5. Compensatory Services were offered, but the parent(s) declined the offer, or the student did not attend. (This is similar to option one except that the services were to be provided after the period of exclusion, not during the exclusion.)
- Letter to the parents that offers services with a response from the parent; Parents response is clearly indicated on the letter.
- Sent home with the student
- Sent via U.S. mail
- Emailed
- Documentation of a follow-up phone call and entered in the SIS parent contact log
- Meeting notes, formal letter returned by the parent, or other written documentation signed by parent declining services.
- Minutes from the IEP meeting where services were offered, and parent declined the services.
- MDR section or meeting minutes reflecting services offered to the student and declined by the parents.
- Notes in parent contact log where parent declined the offer of services.
- Notes / SIS parent contact log with documentation of the multiple attempts made to provide the services.
Example 6: Compensatory Services were provided. (Services were provided outside of the time the suspension occurred. These services were provided after Student Record sign-off and before submitting documentation in the COS application.)
- Letter to the parents that offers services with a response from the parent; Parents response is clearly indicated on the letter.
- Sent home with the student
- Sent via U.S. mail
- Emailed
- Documentation of a follow-up phone call and entered in the SIS parent contact log
- Meeting notes, formal letter returned by the parent, or other written documentation signed by parent agreeing to the services.
- Minutes from IEP or MDR meeting where services were offered and agreed upon by the LEA and parent/student.
- Teacher log showing dates/times/work completed.
- Attendance log with explanation of what was completed and who provided the service.
Example 7: Compensatory services were not offered and have not been provided. ***
- The documentation submitted does not reflect the offer of services, and none of the documentation (IEP meetings/Manifestation Determination Review) included shows additional services were provided.
- The discipline record shows the student was excluded for more than 10 days and nothing in the supporting documentation shows additional services were provided.
- The attendance record also shows the student was excluded for more than 10 days.
- The documentation provided does not match the required services.
***This will represent a finding of noncompliance for the LEA.
