Continuation of Services
Examples for each submission are listed below; it is not an exhaustive list.
- 1. Continuation of services was offered, but the parents declined the offer, or the student did not attend.\
- Letter to the parent that offers services during the period of exclusion with a response from the parent; Parents response is clearly indicated on the letter.
- Sent home with the student
- Sent via U.S. mail
- Emailed
- Documentation of a follow-up phone call and entered in the SIS parent contact log
- Meeting notes, formal letter returned by the parent, or other written documentation signed by parent declining services
- Minutes from IEP meeting where services were offered, and parent declined the services.
- MDR section or meeting minutes reflecting services offered to the student and declined by the parents.
- Notes in parent contact log where parent declined the offer of services
- Documentation that the parent accepted (parent signature on documentation of offer).
- Parent contact log showing attempts to reach the parent to remind him/her of the offer.
- Teacher attendance log showing the student did not attend or teacher log of attempts to reach the student and provide the service.
- 2. Continuation of services was provided but not reported. * (Services were provided during the period of exclusion but not documented with a “Y”- in the SIS.)
- Letter to the parents that offers services with a response from the parent; Parents response is clearly indicated on the letter.
- Sent home with the student

- Emailed
- Documentation of a follow-up phone call and entered in the SIS parent contact log
- Meeting notes, formal letter returned by the parent, or other written documentation signed by parent
- Minutes from IEP meeting where services were offered, and parent declined the services.

- Documentation of provided services (log of dates/times/work completed).
- * This will result in loss of .5 for timely and accurate data. Inaccurate data was submitted.
- 3. Continuation of services was not offered; the student was not suspended for 11 days.**
- Data reported in SR for the consequence of behavior reflects 11 or more days of suspension.
- After amendment/MDR meeting the IEP team adjusted the number of days, but the record in the SIS system was never adjusted.
- The LEA reported inaccurate data.
* Inaccurate reported data results in a loss of .5 for timely and accurate data for Student Record.
- 4. Continuation of services was not offered, the student was suspended for 11 or more days, but the student was not a SWD or suspected SWD for 11 or more suspension days. **
- Evidence shows the student was never a student with a disability.
- Eligibility paperwork showing the student is not eligible is provided.
- Suspension dates occurred outside of the eligibility determination period.
- The student was not considered a student with disability at the time of the suspensions over 10 days.
- Consent for evaluation and attendance/discipline records match
- The student was dismissed prior to receiving over 10 days of suspension.
- Dismissal paperwork and attendance/discipline record match.
- Revocation provided shows the student was no longer a student with disability when some OSS days occurred.

** A student is suspected to be a student with disability if: i) the parent expressed concern in writing to supervisory or administrative personnel or teacher that the child was in need of special education services, ii) the parent requested an evaluation for special education, iii) the teacher of the child or other personnel expressed specific concerns about a pattern of behavior of the child to the special education director or supervisory personnel of the LEA.
5. Compensatory services were offered, but the parents declined the offer, or the student did not attend.

- Notes in parent contact log where parent declined the offer of services.
- Notes showing multiple attempts were made to provide the services.
6. Compensatory services were provided (Services were provided outside of the time the suspension occurred.)
- Letter to the parents that offers services with a response from the parent; Parents response is clearly indicated on the letter.
- Sent home with the student

- Emailed
- Documentation of a follow-up phone call and entered in the SIS parent contact log
- Meeting notes, formal letter returned by the parent, or other written documentation signed by parent agreeing to the services.
- Minutes from IEP or MDR meeting where services were offered and agreed upon by the LEA and parent/student.
- Teacher log showing dates/times/work completed.
7. Compensatory services were not offered and have not been provided. ***
- The documentation submitted does not reflect the offer of services, and none of the documentation included shows additional services were provided.
- The documentation provided does not match the required services.
***This will represent a finding of noncompliance for the LEA.
