Federal Requirements
The monitoring priorities and indicators of the Office of Special Education Programs (OSEP) State Performance Plan (SPP) mandate that youth with IEPs aged 16 and above have an “IEP that includes appropriate measurable postsecondary goals that are annually updated and based upon an age-appropriate transition assessment, transition services, including courses of study that will reasonably enable the student to meet those postsecondary goals, and annual IEP goals related to the student’s transition service needs. There also must be evidence that the student was invited to the IEP Team meeting where transition services are to be discussed and evidence that, if appropriate, a representative of any participating agency was invited to the IEP Team meeting with the prior consent of the parent or student who has reached the age of majority.”
As a compliance indicator (Indicator 13), the target for all LEAs is 100%. To meet the requirements of the SPP Indicator 13, LEAs participate in the Transition Planning review during Cross Functional Monitoring (CFM). Results are included in the LEA’s Determination.
LEAs that earn less than 100% on the initial review are provided another opportunity to earn 100% and update their FYXX Annual Performance Summary status for Indicator 13. This opportunity is the Secondary Transition Planning review. It is available annually to LEAs who were found noncompliant with transition plans.
Indicator 13 Calculation

The target is 100%.
Additional information about the calculation includes:
- IEPs are randomly selected by GaDOE.
- The numerator is the number of compliant IEPs evaluated in the process, and the denominator is the total number of IEPs selected for evaluation.
Reporting LEA’s Score
- The most recent score from CFM or Secondary Transition for each LEA must be reported and the year of that data will be displayed (i.e., 2024-60%).
- The Local Educational Agencies (LEA) Annual Performance Summary containing a target for Indicator 13 (Secondary Transition) is posted in the Special Education (SE) Dashboard in the LEA’s Determination tile and on the Annual Reports and Data Analytics tab on the GaDOE website. The public may view the State’s and any LEA’s Special Education Annual Reports.
Data Source
Indicator 13, Secondary Transition data is collected from either the Cross Functional Monitoring (CFM) application or the Secondary Transition review application. Both are located inside the Special Education (SE) application dashboard. GaDOE reviews the transition plans and reports whether the LEA meets or does not meet the regulatory requirements of 34.C.F.R 300.320; 34. C.F.R. 300.43.
Any score earned during the CFM review will be included in the LEA’s District Determination for the following year (For example, if a LEA participated in CFM in the FY25 school year and earned 80%, the FY26 determination would show 80% for Indicator 13).
The Compliance Rating for the LEA is displayed in the Annual Performance Report (APR) and is also considered as a component of the LEA’s determination.
- ≥ 95% compliance earns 2 points,
- 75%-94% compliance earns 1 point, and
- < 75% compliance earns 0 points.
The LEA compliance rating percentage remains posted in the LEA’s APR until the next CFM Review or the next time the LEA participates in the Secondary Transition Plan review. Participation in the Secondary Transition Plan is required during the year following a LEA’s CFM review if the LEA score was less than 100%.
- The review contains the same elements as the CFM review.
- Any score earned from the Secondary Transition review will replace, for better or worse, the LEA’s Secondary Transition score (Indicator 13) for the following year’s determination.
- For example, ABC County participated in CFM during the FY24 school year and earned 80% for their transition plans. The FY25 District Determination will show ABC County’s Indicator 13 = 80% = 1 point. Subsequently, ABC County participated in the Secondary Transition Plan review during FY25 school year and scored 100%. The 100% will not show on their District Determination until FY26 Determinations are released. Then Indicator 13, Secondary Transition, will show 100% until the LEA is reviewed again for CFM.
Secondary Transition Plan Review
Indicator 13 is initially reviewed during the Cross Functional Monitoring (CFM) review process. LEAs who score less than 100% in CFM participate in the Secondary Transition review the following year. The Secondary Transition review provides another opportunity to meet the 100% target score for Indicator 13.
During the Secondary Transition plan review, GaDOE staff examines the transition plans of randomly selected, pre-identified students using the Transition Documentation Checklist and determine if the transition plans contain all appropriate components. The same number of plans are reviewed. The transition components include:
- Student is invited to the meeting;
- Post-secondary goals based on transition assessments;
- Course of study to facilitate movement to postschool;
- Goals for employment, education/training, and independent living (if appropriate);
- Measurable transition goals;
- Transition services and activities that relate to the transition goals;
- Agency representative invited (if appropriate); and
- Parental consent for agency participation (if applicable).
After GaDOE’s review of the plans, the results are posted in the Secondary Transition SE Applications. If the LEA is found to be 100% compliant, their Indicator 13 results will be updated with the next Determination.
If the GaDOE discovers noncompliant plans, an amendment should be considered as soon as possible. The LEA will also be required to complete a Corrective Action Plan (CAP) which includes components for individual correction of noncompliance (Prong 1) and demonstration of systemic compliance (Prong 2).
