Parent Consent for Provision of Special Education and Related Services
- In order to be consistent with the language in IDEA, GA is no longer using the term Consent for Placement, but instead is using the term Consent for Provision of Special Education and Related Services.
- There has ALWAYS been a requirement that you should not serve a student in special education until you have a signed consent giving permission to do so. This is a need whether this is an initial placement or not. But it is only reported for new SWDs in GA.
- In the past, the IEP was developed and FAPE was offered prior to asking a parent for consent to provide services. However, we were advised by OSEP that we should not require the development of an IEP or SP if a parent does not want their child to be served at all. Therefore, we have revised the procedure for obtaining this consent.
- A district is not required to develop an IEP or SP if a parent is certain they are not interested in their child receiving services.
- A parent is not required to sign consent for services prior to the development of the initial IEP. An IEP or SP may need to be developed for a parent prior to requiring them to sign the consent form.
- Parents should be made aware that the Consent for Provision of Special Education and Related Services is not consent for only those services offered in the initial IEP but is consent for the special education and related services recommended by the IEP team in all future IEPs unless the parent revokes consent.
- This also applies to transfer students. You may use a consent from a prior district (in or out of GA) if you have a copy of that signed consent. If not, you should obtain signed consent as soon as possible.
The Event 14 may be entered on the Timeline Override when setting up a new student in GO-IEP or it can be entered later on the Timeline Page in the Consent for Services box.

- If a parent wants services under an SP, DO NOT have the parent sign IEP Consent for Provision of Special Education and Related Services and indicate NO. Instead, have them sign the SP Consent for Provision of Special Education and Related Services and indicate YES.
- It is not necessary to have either form signed again (for example if the student moves from being served through an IEP to an SP and back to an IEP).
- In Student Record, GA has added this Consent for Services as a new Event 14.
- Event 14 will be required for any NEW SWD student after 6/15/2019.
- It is not necessary to enter the historical date for students previously reported in GA as SWDs.
STAFF WILL NEED TO BE TRAINED TO ENTER THIS DATE & IT SHOULD BE MONITORED CAREFULLY TO BE SURE IT IS ENTERED IN GO TO AVOID SR ERRORS!
- A student is a NEW SWD if they have not been reported in GA with a disability in a prior year, or if they are being re-referred after exiting special education.
- For transfer student who have never been reported as an SWD in GA and are NEW SWDs, an Event 14 will need to be reported.
- The first Consent for Provision of Special Education and Related Services form signed (IEP or SP consent for provision of services) will be captured as an SR Event
- SR Event 12 indicates that the Parent Response is NO.
- must be on or after the Event 04 Initial Eligibility date
- might be prior to OR after the Event 05 Initial IEP date
- would not be reported with an Event 06 (Initiation of Sp Ed Services).
- SR Event 14 indicates that the Parent Response is YES.
- must be on or after the Event 04 Initial Eligibility date
- might be prior to OR after the Event 05 Initial IEP date
- would be reported with an Event 06 (Initiation of Sp Ed Services) unless the Event 06 is a future date to be reported the following year
Staff should upload the signed and dated Consent for Provision of Special Education and Related Services to the documents tab. This will help ensure that any GO LEA serving the student in the future has access to this form and does not need to get this signed again.
