Some students are eligible to complete an alternate math course sequence in high school.  This form is optional and should only be completed if the IEP committee thinks a student may benefit.  The screen shot below shows the state rule that governs the use of the Math Rubric for students. 



To complete the math rubric, program users will click on each of the blue links listed under "Criteria".  





Program users will answer the questions that appear after each link Yes or No.  If the question is answered "Yes", program users are required to indicate where the weaknesses related to the question are documented in the IEP document





After the criteria questions are completed and  saved, the answers will appear on the rubric screen.  Next program users must complete the Assurance Statement section and save the selections.  Any question with an answer of "Yes" in the assurance statement section will result in the student being  ineligible  for participation in the math rubric. After the assurance statement section is saved, program users will click the "Math Rubric Final Decision" button to see if the student is eligible for the math rubric or not. The math rubric final decision will print at the top of the page. If the final decision is NO, the signature page will not print. Example of completed rubric below: 

 




After finalizing a decision, the rubric can be re-opened if needed to make changes using the RE-OPEN button.



Once completed, the math rubric should continue to be completed annually.  

If the student is eligible to complete an alternate math course sequence, math should be included as an area of weakness within the present level even if the student will not be taking math courses during the current IEP period.

  • The answer to the impact question within the present level related to math should say “YES”, and the team would have to decide how the weakness is going to be addressed.
  • There might be times when you say that you will not have goals/service because you have prioritized other needs.   
  • Even in those cases – still say YES to that question in the rubric and then document in the minutes that the math goals and services were in the prior IEP and not in the current IEP because you have prioritized other needs. 
  • Whenever possible, it would be a good practice to have math goals which might be addressed in various services. 
  • If you are addressing the goal in any service, then in the question about services/goals respond with YES and select both goals/services.
